Facility Closures: Two scheduled closures per year aligned with local Spring Break and Fall Break

Golden Years ADHC is a Kentucky-based provider serving seniors and adults through Adult Day Health Care (ADHC), Attendant Care, Case Management, and Non-Emergency Medical Transportation (NEMT) under Kentucky Medicaid and the Home and Community Based (HCB) Waiver program. A dedicated Case Manager Supervisor and Transportation Director lead our Case Management and Transportation service lines — the Administrator works together with both to keep our participants at the center of everything we do.

About the Role

We are hiring an Administrator — a clinically licensed leader (Registered Nurse required) who will own the day-to-day operation of our ADHC center and oversee Attendant Care service delivery, while partnering closely with our Case Manager Supervisor and Transportation Director to deliver a seamless participant experience.

This is the senior on-site leadership seat. You will set the tone, protect the standard of care, keep us fully aligned with Kentucky Medicaid and HCB Waiver regulations, and build a team that people are proud to be part of.

What You'll Do (Areas of Responsibility)

    1. Operational Leadership

Direct the daily operation of the ADHC facility and oversee the Attendant Care service line.

Designate and supervise the staff responsible for dietary and activity services, as required by Kentucky ADHC regulations.

Maintain appropriate staffing ratios, schedules, coverage plans, and a designated alternate when you are absent.

Manage budgets, supplies, vendor relationships, and facility safety/sanitation.

    2. Clinical & Participant Care Oversight

Provide clinical leadership over participant intake, assessment, plan of care, service delivery, and discharge/transfer decisions.

Ensure every participant has a current, person-centered care plan that is reviewed and updated on schedule.

Monitor health, safety, and welfare of participants and respond rapidly to changes in condition.

Ensure timely transfer or referral when a participant's needs exceed the scope of ADHC services and notify the applicable Medicaid waiver program.

    3. Medicaid & HCB Waiver Compliance

Maintain full compliance with the Commonwealth of Kentucky Medicaid program and applicable regulations, including:

902 KAR 20:066 — Operation and Services; Adult Day Health Care Programs

907 KAR 1:160 — Home and Community Based Waiver Services

907 KAR 1:170 — Reimbursement for HCB Waiver Services

910 KAR 1:220 — Programs for the Elderly

Always keep the agency survey ready for unannounced inspections by the Division of Health Care and annual monitoring surveys conducted on behalf of the Department for Medicaid Services.

Oversee accurate documentation, prior authorizations, MWMA submissions, level-of-care recertifications, and Medicaid billing integrity.

Manage license renewals (annual) and ADC certification renewals (every 2 years) without lapse.

    4. Cross-Service Partnership & Care Coordination

Partner with the Case Manager Supervisor to ensure participant plans of care, level-of-care recertifications, and clinical needs are aligned across services.

Partner with the Transportation Director to coordinate participant scheduling, attendance, and any clinical or safety considerations that affect transport.

Serve as the clinical point of contact across service lines for changes in participant condition, incidents, and complex care decisions.

Maintain strong working relationships with the Department for Community Based Services (DCBS), Department for Aging and Independent Living (DAIL), Department for Medicaid Services (DMS), Managed Care Organizations (MCOs), and community resources to advocate for our participants.

    5. Attendant Care Oversight

Oversee Attendant Care service delivery, caregiver assignments, supervisory visits, and documentation.

Ensure attendant care plans align with each participant's person-centered plan of care.

    6. People, Culture & Training

Recruit, onboard, train, evaluate, and retain a team that reflects our values.

Lead new-employee orientation, ensure CPR/First Aid certification, and coordinate required in-service training (including prevention, identification, and reporting of abuse, neglect, and exploitation).

Attend educational programs appropriate to the role and arrange ongoing professional development for staff.

Build a culture where The Golden Rule is The Standard — every day, with every participant, with every teammate.

    7. Quality, Risk & Reporting

Lead quality assurance, incident reporting, corrective action plans, and root-cause follow-through.

Develop and maintain policies, procedures, and emergency preparedness plans.

Report performance, occupancy, compliance, and financial KPIs to executive leadership.

Job Requirements

Required

Active, unrestricted Kentucky license as a Registered Nurse (RN) issued by the Kentucky Board of Nursing.

Current CPR certification (American Heart Association or American Red Cross).

Minimum 2+ years of administrative/leadership experience in healthcare, long-term services and supports, home health, ADHC, HCB Waiver services, or a closely related setting.

Working knowledge of the Kentucky Medicaid Home and Community Based (HCB) Waiver program and applicable Kentucky Administrative Regulations.

Experience supervising staff, managing budgets, and leading regulatory inspections/audits.

Proven ability to collaborate cross-functionally with peer leaders (e.g., case management and transportation directors) to deliver a seamless participant experience.

Valid Kentucky driver's license, reliable transportation, and ability to pass a comprehensive background check, drug screen, and TB screening.

Strong written and verbal communication, documentation, and computer proficiency (EHR, MWMA, Microsoft Office/Google Workspace).

Strongly Preferred

Bachelor's or higher degree (BSN, MSN, or healthcare administration).

Prior experience as an Administrator, Director of Nursing, or Program Director in ADHC, PACE, Personal Care, Home Health, or HCB Waiver settings.

Experience with Kentucky MCOs (Humana, Anthem, Passport, Aetna Better Health, WellCare, UnitedHealthcare Community Plan).
