Job description

Position Summary

Kadima Healthcare Rehabilitation and Nursing is seeking a dedicated Licensed Practical Nurse (LPN) to join our team. In this role, you will provide direct patient care in accordance with physician orders and established care plans while working collaboratively with physicians, RNs, and the interdisciplinary team to support residents’ physical, emotional, and overall well-being.

Key Responsibilities
  • Assess residents’ health status through observation, physical assessment, and review of diagnostic information

  • Administer medications and treatments in accordance with physician orders

  • Participate in care planning and collaborate with the interdisciplinary team

  • Monitor, document, and communicate changes in resident condition to physicians and nursing leadership

  • Supervise and support nursing assistants and other care staff

  • Educate residents and families on health conditions and care plans

  • Complete required documentation accurately and in compliance with regulations and facility standards

Qualifications
  • Current licensure as a Licensed Practical Nurse (LPN)

  • Active Pennsylvania license (or eligibility)

  • Strong clinical, communication, and organizational skills

  • Experience in long-term care or skilled nursing preferred, but not required

  • Compassionate, patient-centered approach to care
