Job Summary:


The Outreach Coordinator will facilitate care coordination to patients and patients' families enrolled in care management services and assist with data management and reporting for care management services. The Outreach Coordinator will also assist in coordinating referrals to self-management education and support services and provide outreach for population health management.


Key Responsibilities: 

  • Data management for care management services. Maintain reports received from Mo HealthNet Primary Care Health Home (PCHH). Complete and submit monthly PCHH attestation.
  • Provide outreach to potential enrollees for PCHH and other care management services. Submit monthly enrollment, discharge, and transfer forms to Mo HealthNet PCHH.
  • Use care opportunity reports, health technology tools, and registries to provide outreach to patients and patients' families enrolled in care management services. Provided proactive reminders to patient populations to address a variety of health needs. 
  • Connect patients and patients' families to community resources and internal services. 
  • Communicate with patients and patients' families in person (clinic or community), by phone, or through patient portal.
  • Facilitate or schedule medical/dental appointments. Determine transportation needs and assist in arranging transportation to      medical/dental appointments. 
  • Assist with patient(s) care transitions to primary care from hospitals, emergency departments, and other settings.
  • Identify individuals with high utilization of emergency rooms and/or inpatient care. Coordinate care with patients, families, and other healthcare team members to identify utilization patterns and provide intervention(s) to reduce inappropriate or preventable visits.
  • Promote individual and/or group health education and chronic disease self-management education. Coordinate logistics for patient education classes. Coordinate referrals to individual and group patient education.
  • Build relationships and interact professionally with community partnerships, health, and social service providers.
  • Document in electronic health record appropriately and in a timely manner. 
  • All other duties as assigned.

Required Qualifications:

Preferred Qualifications:

  • Bachelor's Degree in a health related field preferred.
  • Medical Assistant or Licensed Practical Nurse license preferred.
