Gurwin Healthcare System is a full continuum of care providing extraordinary care and caring to the people that we serve. Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis. Sharing the campus is Gurwin’s Assisted Living community (Gurwin Jewish ~ Fay J. Lindner Residences), our independent living community (Fountaingate Gardens), and our Certified and Licensed Home Care Agencies.

Our innovative philosophy and growing services offer opportunities to advance your career. We are currently seeking a Social Worker for our Home Care Division with adult experience who will embody the passion and commitment required to deliver the ultimate care for our patients.  

EDUCATION AND EXPERIENCE REQUIREMENTS:

Masters Degree in Social Work from a school accredited by the Council on Social Work Education and the Department of Education preferred

Certification by the Department of Education of New York State to practice Social Work.

Minimum of one year of social work experience in a health care setting as a member of a multi-disciplinary team, is required.

Home Health Care experience is preferred.

Able to make clinical and managerial decision within the scope of professional practice.

PHYSICAL WORK REQUIREMENTS:

Ability to travel to patients' homes to make home visits.

Able to lift, pull, push, and manipulate patients and equipment equal to or more than physical demands.

PRIMARY RESPONSIBILITIES:

Assesses the relationship of the client’s medical and home situation and financial resources.

Collaborates with the Community Health Nurses in developing the plan of treatment.

Provides direct social work services to patients/families and makes community referrals as appropriate.

Provides concrete social work services to enable patients to receive entitlement they may be eligible to receive or to assist in resolving social or emotional problems that may have an adverse effect on the client’s ability to respond to treatment.

Prepares clinical and progress notes for the patient record. Directs copies to be sent to other appropriate agencies.

Writes all social work correspondence for program cases.

At the point of termination of CHHA social work services, transfers cases to appropriate social agency if need to continued service exists.

Participates in meetings and conferences as needed.

Performing related duties, as necessary.

Adhering to all aspects of Customer Service, some examples include the following: Working together, showing empathy, courteous communication, respect of others.
