Key Responsibilities
  • Perform comprehensive psychosocial assessments for clients, including individuals, couples, families, adolescents, and groups, to identify needs and develop tailored treatment plans.
  • Conduct initial evaluations to triage patient care effectively, ensuring appropriate interventions are identified and implemented.
  • Utilize documentation and conduct case discussions during team meetings to ensure effective treatment planning and coordination with other healthcare providers.
  • Demonstrate a strong understanding of the multifaceted needs of patients, including physical, psychological, social, educational, and safety requirements.
  • Manage a caseload with an emphasis on providing pertinent case management services, involving collaboration with external agencies and community resources to enhance client care.
  • Complete ethical and timely documentation aligned with organizational standards.
  • Implement Cognitive Behavioral Therapy (CBT) and other evidence-based practices to support patients in achieving individual service plan goals.
