This position is designed to be fully remote, utilizing telehealth platforms to reach patients effectively. • Perform comprehensive psychosocial assessments for clients, including individuals, couples, families, adolescents, and groups, to identify needs and develop tailored treatment plans.
• Conduct initial evaluations to triage patient care effectively, ensuring appropriate interventions are identified and implemented.
• Utilize documentation and conduct case discussions during team meetings to ensure effective treatment planning and coordination with other healthcare providers.
• Demonstrate a strong understanding of the multifaceted needs of patients, including physical, psychological, social, educational, and safety requirements.
• Manage a caseload with an emphasis on providing pertinent case management services, involving collaboration with external agencies and community resources to enhance client care.
• Complete ethical and timely documentation aligned with organizational standards.
• Implement Cognitive Behavioral Therapy (CBT) and other evidence-based practices to support patients in achieving individual service plan goals.

The population served includes a wide range of clients facing various mental health challenges. • Active licensure as a Licensed Marriage and Family Therapist (LMFT), Licensed Mental Health Counselor (LMHC), or Licensed Clinical Social Worker (LCSW) in the state of New York.
• A minimum of a Bachelor's degree in psychology, social work, counseling, or a related field is required.
• Availability to work between 10 to 40 hours per week, accommodating a flexible schedule for both patients and personal commitments.

• A minimum of two years of clinical experience in mental health counseling, with a proven track record of providing high-quality care.
• Experience providing telehealth services and familiarity with digital health tools is highly desirable.
