Job description

Jail Coordinator - Navajo County
Community Bridges, Inc. (CB I) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. CBI is the premiere non-profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona, including prevention, education and treatment using cutting-edge, nationally recognized, evidence-based models.

Job Summary 
The Community Bridges Inc. (CBI) Jail Coordinator is responsible for delivering treatment, and discharge planning services to identified Gila County Jail Program Patients. Jail Coordinator is responsible for assisting inmates with jail release planning and coordinating into behavioral health services.  Jail Coordinator will participate in a weekly staffing with probation and Jail staff to provide updates on each patient and coordinate care. Each are below is defined by the specific function as it relates to the policy, process, and the governing rules set forth by the Department of Health Services (DHS), Regional Behavioral Health Authority (RBHA), Arizona Complete Care (ACC) Office of Behavioral Health Licensure (OBHL), and the Code of Federal Regulations (CFR). 

Education and Experience Required

  • 2 years related experience in behavioral healthcare, physical health or substance abuse.
