Job description

We have immediate opportunities on our Transition Team for Registered Nurses / RNs interested in helping infants, children, and adults receive complex care at home throughout Chester County, Delaware County and Montgomery County! The role is FT – 40 hours per week. This team will support the transition of care from the hospital team to ongoing home care services. This role offers a unique opportunity to learn how to care for clients with rare and complex diseases. As part of the Transition Team, you will be responsible for delivering quality care to clients and providing extensive training to the ongoing care team.

Present openings are day shifts (Monday through Sunday) and overnight shifts (Monday through Sunday). You choose which shift works best for you!

Qualifications for Transition Team Registered Nurse / RN

  • NICU, PICU, ICU, or home care trach/vent experience required
  • Achieve a passing score on high-tech Adult and Pediatric simulations
  • Hold current CPR certification
  • Ability to work in a teaching setting to prepare and educate the ongoing BAYADA care team
  • Availability to commit to an ongoing, consistent schedule of 40 Hours per week
  • Flexibility to cover weekends - at least four weekend shifts per month
  • Ability to travel throughout Chester County, Delaware County and Montgomery County (only southern and western parts of MontCo)
