Job description

We have immediate opportunities on our Transition Team for Registered Nurses / RNs interested in helping infants, children, and adults receive complex care at home in Chester and Montgomery County. The role is FT – 40 hours per week. This team will support the transition of care from the hospital team to ongoing home care services. This role offers a unique opportunity to learn how to care for clients with rare and complex diseases. As part of the Transition Team, you will be responsible for delivering quality care to clients and providing extensive training to the ongoing care team.
Present openings are day shifts (Monday through Sunday) and overnight shifts (Monday through Sunday). You choose which shift works best for you!

Qualifications for Transition Team Registered Nurses / RNs:
  • Have a current license as a Registered Nurse / RN
  • NICU, PICU, ICU, or home care trach/vent experience required
  • Achieve a passing score on high-tech Adult and Pediatric simulations.
  • Hold current CPR certification
  • Ability to work in a teaching setting to prepare and educate the ongoing BAYADA care team
  • Availability to commit to an ongoing, consistent schedule
  • Ability to travel throughout Chester, Delaware, and part of Montgomery County
  • Ability to work around pets
Rate is based on qualifications and experience for the Transition Team role.
Transition Team rates: $37-$42
Standard rates (non-Transition team): $32-$35
