Job Summary
The Care Team RN is a critical clinical leader within our client's primary care team. Serving as the primary liaison between patients and providers, the RN integrates advanced clinical judgment with efficient care coordination. This role is designed for a Registered Nurse who excels at high-level triage, complex patient education, and overseeing the clinical competency of the medical support staff to ensure seamless, high-quality care delivery.
Key Responsibilities
  • Advanced Clinical Triage: Utilize the Nursing Process to assess and triage patients for the appropriate level of care. You are the clinical "air traffic controller" for both scheduled visits and emergent walk-ins.
  • Care Management & High-Risk Outreach: Identify and support "rising risk" patients. You will develop care plans that address physical, psychosocial, and social determinants of health.
  • Clinical Mentorship: Support the professional development of the clinical team by assessing and validating the skills and competencies of Medical Assistants (MAs).
  • Provider Partnership: Work in tandem with a dedicated team of 3 APPs and 1 Physician. You will handle complex paperwork, specialty referrals, and standing order protocols to optimize provider efficiency.
  • Patient Empowerment: Provide expert education on complex disease processes (e.g., diabetes, hypertension) and medication regimens, fostering patient engagement and self-management.

Qualifications
  • Current Colorado RN License.
  • Current CPR (BCLS) certification.
  • Graduate of an accredited Nursing Program required; BSN preferred.

Experience
  • Two years' experience in case management and clinical nursing preferred.

Skills
  • Computer Skills: Basic proficiency in accessing the intranet/internet, managing timecards, reviewing policies and procedures, communicating via email, and using Microsoft Office Suite.
  • Epic proficiency preferred.
  • Spanish Bilingual preferred.
  • Experience with Medicare/Medicaid population and paperwork preferred, as they make up 55% of the patient base.
  • Care Management/Case Manager experience preferred.

Additional Requirements
This position involves working a flexible schedule with either four 10-hour shifts or five 8-hour shifts during weekdays.
