Title: Driver, per diem- Suffolk Country & The Hamptons
Reports To: Logistics Coordinator
Supervises: Account Manager, Waiver Programs
JOB SUMMARY:
The Driver is responsible for the safe, reliable, and on-time transportation of home health aides between central pickup locations and client homes across assigned routes. This role is essential to ensuring uninterrupted patient care by getting the right aides to the right locations on schedule every day. The Driver operates with professionalism, accountability, and a strong commitment to safety, serving as a key link in ACH's care delivery chain.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Safely transport home health aides from designated pickup points to client homes along assigned routes, ensuring on-time arrival for every scheduled shift.
  • Follow daily passenger manifests accurately, confirming aide pickup and drop-off at correct locations and communicating any discrepancies to the coordination team promptly.
  • Monitor traffic, road conditions, and route efficiency in real time, adjusting as needed to maintain schedule integrity and minimize delays.
  • Communicate proactively with the Logistics Coordinator regarding delays, aide no-shows, vehicle issues, or any circumstances that may impact shift coverage.
  • Conduct pre-trip and post-trip vehicle inspections, reporting any maintenance needs, safety concerns, or incidents immediately and in accordance with company policy.
  • Maintain a professional, respectful, and courteous demeanor with all aides, staff, and any individuals encountered while representing Always Compassionate Health in the field.

QUALIFICATIONS/EDUCATION/LICENSURE/CERTIFICATIONS:
  • Valid New York State driver's license with a clean driving record.
  • Prior experience in passenger transportation, shuttle operations, or a similar driving role preferred.
  • Strong knowledge of Long Island roadways and traffic patterns, particularly across Hamptons and Suffolk County.
  • Ability to follow schedules, manifests, and dispatch instructions with precision and reliability.
  • Strong communication skills and ability to coordinate in real time with dispatch and coordination staff.
  • Punctual, dependable, and able to maintain composure under time-sensitive conditions.
  • Offer contingent on MVR completion
  • Defensive Driving Course and/or Driving Safety Training required

Work Environment:
The Driver operates primarily in a vehicle-based environment, spending the majority of working hours on the road across assigned Long Island routes, primarily Suffolk County and the Hamptons. This role involves early morning start times, varied weather and traffic conditions, and a schedule that is directly tied to patient care needs. The position requires physical readiness to operate a vehicle safely for extended periods and the flexibility to respond to real-time changes in routing or scheduling.
Compensation Range
  • $22-24/hour
