Key Responsibilities
  • Conduct comprehensive patient evaluations that include reviewing medical histories and performing specific assessments to establish effective treatment plans.
  • Communicate evaluation results and treatment strategies clearly to patients and their families, ensuring they are involved in setting mutual health goals.
  • Implement treatment protocols tailored to the individual patient’s needs, continuously adapting based on their health progression and the latest evidence-based practices.
  • Collaborate with other healthcare professionals to coordinate patient care, documenting any necessary adjustments to treatment plans and seeking additional consultations as required.
  • Manage and document all aspects of patient care including progress notes, discharge plans, and follow-up care, ensuring compliance with all healthcare regulations.
  • Exhibit exemplary time management by maintaining an organized schedule, ensuring you meet individual productivity targets while attending required meetings.
  • Engage in departmental goal-setting and quality improvement initiatives, contributing to both organizational success and superior patient care.
  • Seek and participate in continuing education opportunities to enhance personal professional development and contribute to the educational growth of interns and students.
  • Deliver exceptional customer service, ensuring patients feel valued and supported throughout their care journey.
  • Participate in marketing initiatives to promote services, aiming to increase community awareness and access to our clinical services.
Patient Population / Client Focus

As a Home Care Physical Therapist, you will work with a diverse patient population that includes individuals recovering from surgeries, managing chronic pain, and addressing mobility challenges. You'll focus on rehabilitating patients in the comfort of their homes, allowing you to create personalized care plans based on their unique living situations and therapy needs.

Qualifications
  • Bachelor’s degree in Physical Therapy.
  • A valid Physical Therapy license in the State of Texas.
  • Current Basic Life Support (BLS) certification from the American Heart Association.
  • Strong communication and interpersonal skills to interact effectively with patients and their families.
  • Ability to work independently while remaining part of a collaborative team environment.
Preferred Experience
  • A minimum of 2 years of experience in physical therapy, with a preference for those who have experience in a home care setting.
  • Previous work in patient education, rehabilitation planning, and interdisciplinary teamwork is highly valued.
  • Familiarity with patient documentation systems and health information technology.
Compensation and Benefits
