Array Wellness Center PC is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in a full-time, remote position. This role is structured to provide maximum flexibility, allowing you to work remotely in a climate-controlled setting while still engaging actively in team collaboration. As a part of our community health clinic, you will interact frequently with healthcare providers and colleagues to ensure comprehensive care for our patients. Daily responsibilities may include video consultations, case reviews, and real-time collaboration on treatment plans.

Key Responsibilities
  • Conduct thorough assessments and evaluations of clients’ mental health, employing tools such as the Diagnostic and Statistical Manual of Mental Disorders (DSM) to inform diagnoses and care plans.
  • Develop individualized treatment plans in partnership with patients and their families, adapting strategies based on their unique needs and goals.
  • Continuously monitor patient progress and adjust interventions to promote symptom reduction and ensure safety; using therapeutic modalities appropriate for each case.
  • Coordinate referrals and resources, acting as a vital link between patients and external treatment providers to facilitate seamless care transitions.
  • Maintain accurate and timely clinical documentation of treatment records and reports, adhering to state regulations and departmental protocols.
  • Engage in mentoring students and interns, offering guidance that enhances their professional development and aligns with best practices in social work.
  • Collaborate with clinical supervisors for peer reviews and participate in ongoing training to refine clinical skills and enhance team performance.
Patient Population / Client Focus
