Key Responsibilities

  • Collaborate with various healthcare professionals to formulate and implement effective psychosocial treatment plans, ensuring seamless referrals to specialized services, entitlement programs, and other necessary resources.
  • Support patients and families in coping with the psychological ramifications of illnesses and life changes, providing crisis intervention and grief counseling as required.
  • Manage a diverse caseload, independently prioritizing cases based on clinical acuity and patient needs.
  • Engage in proactive case finding strategies to identify patients and families requiring additional psychosocial support.
  • Attend interdisciplinary rounds and team conferences to stay updated on patient conditions and collaborate on treatment strategies.
  • Deliver psychoeducation to patients and their families, assisting them in adjusting to new diagnoses and treatments, and supporting transitions, especially for youth moving into adult services.
  • Contribute to quality assurance initiatives aimed at enhancing social work practices and patient care, partaking in the development of policies and procedures that guide behavioral health services.
