●      About Facility:

o   Community based hospital in urban area, High Point, NC

▪       304 licensed beds

▪       Teaching Status: Interns, Residents, APPs, Nursing Students

▪       Chest Pain Center Accredited from American College of Cardiology

▪       Stroke Center designated by TJC: Primary Stroke Center

●      Unit Information:

o   Beds: 20

▪       Average Daily Census = 18

o   Minimum Years of Experience required? 1 year ICU experience

o   Will you accept a first-time traveler? Yes

●      Patient Types/Common Diagnoses:

o   General medical patients requiring advanced hemodynamics and continuous monitoring of cardiac, respiratory, hemodynamic and neurologic status

o   Respiratory compromise or failure requiring medium or high flow oxygen devices, NIPPV or ventilator support

o   Renal Failure with CRRT capabilities

o   Severe Sepsis with hemodynamic instability and IV resuscitation

o   General, Vascular, or thoracic surgical patients requiring continuous cardiac, respiratory or hemodynamic support include pharmacological vasopressor support

o   Cardiac arrest requiring targeted temperature management

o   Neuro – Medical cases Stroke with or without Tenecteplase (TNK) or Traumatic Fall with some neurosurgical intervention (EVD management or ICP monitoring)

o   DKA/HHNK – Insulin drips with Endotool titrations

●      Patient Ratios:

o   RN: ICU Level of care is 1:2; Special high acuity of RN 1:1 care (organ procurement or CRRT); Overflow patients waiting on stepdown Intermediate the RN to patient ratio is 1:3 or 1:4 depending on interventions.

o   CNA: Varies and we do have Nurse Tech support.

o   Charge Nurse is typically a “free” charge available to resource and assist.

●      Required Licensure & Certifications:

o   BLS, ACLS, NIHSS

●      Skills required:

o   Cardiac and hemodynamic monitoring, cardiac strip and rhythm interpretation, arterial lines, titration of drips, ventilator care (we have Respiratory Therapist 24/7), vascular lines, chest tubes, post procedure site care – trans radial bands, femoral venous or arterial sheaths, CRRT, post pulmonary artery thrombectomy (EKOS, Prenumbra, Inari)

●      Support within the Department:

o   CNA? Yes

o   Receptionist/HUC? Varies – day shift only (8am-5:30pm)

o   Charge nurse? Yes

o   Phlebotomy? Varies

o   IV/VAT Team? No

o   Resource Nurse? No (Charge Nurse serves as Resource)

o   Rapid Response Team? Yes

o   RT 24/7? Yes

o   Hospitalist 24/7? Yes

o   Intensivist 24/7? Yes

o   Telemetry monitoring? The RN is responsible for continuous cardiac monitoring including interpretation of the rhythm

●      Technology/Equipment:

o   EMR: EPIC

o   IV Pump: Baxter

o   Med dispensing: Omnicell

o   Monitors: Philips Intellivue

o   Unit specific equipment: CRRT-NxStage; Edwards Lifescience-Hemosphere/FloTrac for minimally invasive advanced hemodynamics; TTM – Artic Sun

●      Orientation (facility and unit):

o   Orientation – 36 hours week 1 includes NTO and Online CBL (Healthstream)

o   12-hour departmental specific orientation shift with an RN Preceptor

●      Scheduling:

o   Schedule cycle: Every 6 weeks
