Mental Health and/or ID/DD experience is preferred for this position.
Responsibilities and Duties for LPN:
- Implementing and maintaining Individual Service Plans.
- Conducting ongoing physical and mental assessments.
- Maintaining accurate consumer records, monitoring consumer safety.
- Supervising and monitoring individual medical treatment programs.
- Administration and delivery of consumersโ prescribed medication as needed.
- Monitoring and education regarding medication compliance and side effects.
- Assessing consumerโs vital signs, height, weight, medication compliance, possible side effects, and obtaining AIMS score or laboratory tests as ordered by a physician.
- Completing PA (prior authorizations) and patient assistance for consumerโs psychiatric medication needs.
- Compliance with agency policies and procedures, and adherence to all relevant local, state, and federal legal and regulatory requirements.
Qualifications and Skills for LPN:
- Graduation from an accredited Licensed Practical Nurse Program.
- Current Virginia unrestricted LPN License or Current Unrestricted LPN License from a Nurse Licensure Compact State.
- 1-year LPN work experience in a mental health program and/or residential setting.
- 1-Year LPN work experience involving individuals with intellectual disabilities.
- Valid driverโs license with acceptable DMV record.
- Proficiency in Microsoft Office.
- Preferred: Experience using Electronic medical records; basic knowledge of Medicaid/Medicare Regulations.
