Job description

BASIC PURPOSE OF THE JOB

Provides specialized technician support under the direct supervision of a pharmacist by interviewing patients, families, and caregivers to compile accurate, comprehensive, and verified home medication lists for hospital admissions. Medication lists are entered into the electronic health record, and potential discrepancies are reported to a pharmacist.

JOB REQUIREMENTS

Supervisory Responsibilities: No

Minimum Education: High school diploma or GED required

License/Certification Required: Current registration with the TN Board of Pharmacy required. State Board approved Technician Certification for CPhT status required. PTCB Medication History Certificate required within 2 years of hire date.

Minimum Work Experience: 3 years experience as a Pharmacy Technician required.

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
  • Advanced computer skills
  • Use of Drug Diversion software (ex. Pandora or Analytics), Omnicell, and Meditech is heavily preferred
  • Basic math skills
DUTIES AND RESPONSIBILITIES
  • Medication History: Assists pharmacists in obtaining complete and accurate medication histories for patients who are being admitted to the hospital. Conducts patient/family interviews regarding home medications, recognizing and clarifying incomplete or inaccurate lists based on medical history and communication with outpatient pharmacies and physician offices. Enters home medications accurately into EHR in a timely manner for pharmacist review prior to patient admission, or as soon as possible afterwards. Provides continued communication between nursing, physicians, and outpatient pharmacies regarding home medications for patients admitted to the hospital. Promptly brings clarification/intervention issues to the pharmacist's attention for resolution. Demonstrates proficiency with math calculations. Maintains accurate records of medication reconciliation activities and quality assurance. Demonstrates clean and organized work flow, including adherence to infection prevention protocols for patient rooms. All competencies are maintained in a timely manner and do not go past due.
  • Demonstrates respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment. Continually displays a "can do" attitude within the department and across departmental lines to contribute to the overall customer service program in place at the hospital. Resolves questions with the pharmacist, Nursing Service, and/or physician. Promotes and maintains professional relationships with other departments. Assists main pharmacy when required (during staffing or workload emergencies) by appropriately delivering medications to the nursing units. Effectively trains new interns or technicians when assigned. Teaches "shadowing" pharmacy students.
  • Promotes effective working relations and works effectively as part of a department/unit team inter- and intra-departmentally to facilitate the department's/unit's ability to meet its goals and objectives. Fosters mature professional relationships with fellow employees in a courteous, friendly manner as measured by management observation and peer input. Reads, attends, and acknowledges pharmacy and hospital communications. Willingly accepts any other assignment that may be requested.
  • Utilizes hospital resources and time respectfully and accountably.
PHYSICAL REQUIREMENTS

  • Light lifting, pushing and pulling is required for 10-20 lbs occasionally and frequent moving of objects of less than 10 lbs is required. Frequent sitting with some walking, standing, squatting, bending and reaching is required. Keyboard/computer use and/or repetitive motions may be required.
