Job description

VARIABLE   Job Summary: Provide evaluation, diagnosis, evidence based treatment, and prevention measures to patients related to speech, language, learning, cognitive-communication, voice, swallowing/feeding and fluency disorders. Develop and implement patient care plans. Provide patient care within an integrated Care Delivery (IGP) model including coordinated care with physicians, family and other team members with regards to diagnosis, intervention and progress. Fulfill all documentation and coding requirements. Follow Kaiser Foundation Health Plan of Washington and departmental policies and procedures. Comply with all regulatory requirements.
Essential Responsibilities:
  • Evaluate, diagnose and provide treatment for speech, language/learning, cognitive-communication, swallowing and fluency disorders.
  • Complete required procedure or visit documentation and coding.
  • Contribute to program development and quality activities. Meet continuing education, certification and training requirements.
  • Provide feedback to team per integrated Care Delivery model.
