Nestled into the most northern part of Vermont, North Country Hospital is a community-focused 25 bed critical access hospital, where we prioritize the well-being of our neighbors. North Country Hospital is more than just a healthcare provider; we are a vital part of the community, working together to build a healthier future. Our patient's health is our priority, and we go above and beyond to care in every way possible.
Position Statement
The pharmacist is an integral part of the healthcare team. Providing support for physicians, advanced practitioners, and nursing staff to assure patients receive optimal care and education.
Key Responsibilities:
  • Attends discharge planning meeting each morning.
  • Oversees the medication reconciliation process.
  • Collaborates with providers for medication orders.
  • Provides education for patients with new medications or complicated medication reconciliation.
  • Monitors patients receiving total parenteral nutrition (TPN).
  • Responds to all codes, traumas and critical patients, when available.
  • Verifies and checks automated dispensing cabinets as scheduled, such as Omnicell.
  • Participates in on-call rotation with 30-minute response time if needed to be physically in the facility.
  • Supervises the filling and dispensing for the Employee Prescription plan.
  • Reviews all orders for dosing and accuracy in the outpatient treatment center (OTC).

Knowledge, Skills and Abilities:
  • Bachelor of Science Degree/Pharm. D
  • Current Vermont license
  • One year experience or internship in hospital pharmacy
