• Conduct comprehensive assessments of patients to diagnose their mental health conditions and evaluate the effectiveness of their care plans.
• Utilize the Diagnostic and Statistical Manual of Mental Disorders (DSM) for accurate diagnosis and ongoing monitoring of patient progress.
• Create and implement individualized treatment plans that consider the cultural diversity and special needs of the patient population.
• Maintain up-to-date and precise treatment records, reports, and clinical data in compliance with regulatory standards.
• Collaboratively develop treatment strategies with patients and their families, ensuring that their needs and objectives are central to the care plan.
• Provide referrals and act as a liaison between patients and treatment providers, maintaining professional relationships to enhance patient outcomes.
• Deliver education and mentoring to interns, residents, and volunteers, fostering a culture of learning within the team.
• Participate in peer review, clinical collaboration, and support for fellow team members to cultivate professional growth and development.

The primary clientele for this role are individuals and families grappling with the psychological impacts of cancer diagnoses. • Must hold a Master’s degree in Social Work from an accredited institution.
• Possess a current and valid state license as a Licensed Clinical Social Worker (LCSW).

Prior experience working within an outpatient mental health context, particularly focusing on oncology or chronic illness, is highly desirable. Heritage Wellness Clinic offers a competitive salary range of USD 90,000 – 105,000 per year, depending on your experience and qualifications.
