Summary:

The Licensed Clinical Social Worker (LCSW) provides advanced clinical social work services in support of the Emergency Department (ED) at Joe DiMaggio Children's Hospital (JDCH), with expertise in high-risk psychosocial, behavioral health, and legally complex cases in the acute care setting. The LCSW manages a caseload and serves as a clinical expert and resource for cases requiring advanced social work interventions. This role utilizes specialized clinical knowledge and advanced clinical skills to provide comprehensive assessment and intervention related to medical, psychosocial, mental health, and behavioral health needs; patient and caregiver navigation; and discharge planning. The LCSW collaborates with patients, families, and the interdisciplinary care team to address complex needs and support safe, effective transitions of care.
This position would work from 12:00 PM to 12:00 AM, with rotating weekend shifts. The schedule may be subject to change based on departmental needs.

Responsibilities:

May supervise and/or train new staff, students, and interns.Consults and collaborates with multidisciplinary transplant providers. Makes appropriate referrals to community agencies, which include but are not limited to: Department of Children & Families (DCF), Adult Protective Services (APS), Social Security Administration (SSA), Center for Medicare Services (CMS), and/or disease-specific resources.Designs and facilitates support groups and other programs.Interviews and assesses patients, family, caregivers, and/or legal representatives. Performs comprehensive psychosocial assessments for applicable moderate or high-risk patients to assist with care coordination and discharge planning needs.Develops behavioral agreements with patient and/or families that address adherence and commitment to treatment plan. Assists with patient fundraising.Leads complex family meetings. Assists patient in the comprehension of complex instructions, resolves conflicts, or facilitates conflict resolution. Provides crisis intervention for patients and family members.Attends interdisciplinary rounds and identifies appropriate resources for financial, insurance, transportation, housing, healthcare, mental health, and spiritual services.Manages cases involving guardianship, abuse/neglect, ethical concerns, and legal involvement. Documents clinical assessments and interventions.
