Job description

JOB SUMMARY
The PHARMACIST, under general supervision, provides pharmacy services to patients and medical practitioners. Fills prescriptions, dispenses medicines, counsels patients and medical practitioners regarding the nature and use of drugs, and trains and/or supervises pharmacy interns. Prepares and maintains all necessary records on prescriptions, patient charges, and inventory. Services are limited to those in compliance with the facility policies/procedures.

EDUCATION
Completion of accredited university pharmacy program and successful passing of competency exams meeting state regulations.
LICENSURE
Current and unrestricted license to practice as a pharmacist in the state of practice; NAPB examination.
EXPERIENCE
two years of current experience as a Pharmacist in a clinical setting.
CREDENTIALS
Current CPR at least; and other health and screening tests as required by specific facilities and/or regulatory agencies.
ENVIRONMENTAL WORKING CONDITIONS
Various client facility settings; possible exposure to blood, bodily fluids, and other potentially infectious materials.
REPRESENTATIVE DUTIES AND RESPONSIBILITIES
  • Complies with ATC policies/procedures.
  • Complies with client facility policies/procedures.
  • Documents accurately and completely services provided.
  • Maintains confidentiality relative to patient care and facility practices in accordance with the Health Insurance Portability and Accountability Act.
  • Provides patient care in a non-judgmental, non-discriminatory manner that considers cultural diversity and age appropriateness so that autonomy, rights, and dignity are preserved.
  • Communicates information effectively to appropriate personnel.
  • Maintains competency by participating in continuing education programs and meets state specific requirements.
  • Complies with accepted ethical and professional standards of conduct.
  • Possesses knowledge of drugs and their indications, contraindications, dosing, side effects, and proper administration.
  • Prepares and/or oversees the preparation and dispensing of prescription medications to patients or medical practitioners; makes decisions regarding generic substitution of prescribed pharmaceuticals.
  • Counsels patients and/or medical practitioners on drug indications/contraindications, dosage, drug interactions, and side effects; provides other drug and pharmaceutical information as appropriate.
  • Oversees the acquisition and disbursement of drugs and medications to various offices, clinics, and other sites.
  • Provides guidance and training to pharmacy interns during course of work and monitors performance.
  • Establishes and maintains methods and manner of storage and record keeping systems to provide for safekeeping of pharmaceuticals.
  • Takes patient histories as required; makes initial clinical assessments and refers patients to appropriate clinicians.
  • Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards.
