We are seeking a dedicated Part-Time Hospice Social Worker (MSW) to provide individualized hospice support services at the Kate B. Reynolds Hospice Home.

Position Requirements

โœ” Masterโ€™s Degree in Social Work (MSW) required
โœ” Knowledge of community social, financial, and healthcare resources
โœ” Strong written and verbal communication skills
โœ” Ability to work independently and within an interdisciplinary team
โœ” Commitment to hospice philosophy and compassionate care
โœ” Excellent organizational and prioritization skills

Key Responsibilities

As a Hospice Social Worker, you will:

  • Provide individualized hospice services to patients and families
  • Assess emotional, social, and financial needs
  • Develop psychosocial goals within patient care plans
  • Support patients and families in accessing community resources
  • Collaborate closely with the interdisciplinary team (IDT)
  • Attend and contribute to IDT meetings
  • Assist with discharge planning and transitions of care
  • Educate families regarding Hospice Medicare/Medicaid benefits
  • Complete required clinical documentation accurately and timely
  • Support orientation of new staff, interns, and volunteers
  • Participate in Social Work on-call duties
