Wage Range: $24.91 - $39.85 per hour

Posted wage ranges represent the entire range from minimum to maximum. For jobs with more than one level, the posted range reflects the minimum of the lowest level and the maximum of the highest level. Job Summary:


Performs a variety of duties that support Value Based Care through Population Health initiatives. Provides clinical guidance and assists the Population Health & Quality Managers with audits, contract reviews, and improving data collection methods to ensure accuracy. Responsible for using and/or creating reports to identify and abstract clinical data and reconcile outside records to close care gaps, including pending orders when needed. Provides supplemental data for payer measures.

A subject matter expert (SME) to the EHMG clinics and other key stakeholders. Supports clinics with process improvement activities to meet quality metrics and close care gaps by providing patient outreach and education.

Primary Duties:

  1. Work in collaboration with clinical staff and other health care professionals to ensure continuity and coordination of care for patients and families to help meet patient goals.
  2. Supports clinical staff with implementing processes for closing care gaps and meeting quality metric goals.
  3. Educate and train staff in data collection processes and techniques.
  4. Analyze contract performance for clinic care team opportunities.
  5. Review target patient lists or disease registries to identify low-and emerging-risk patients.
  6. Conduct chart review and outreach to low and rising risk patients to eliminate identified care gaps.
  7. Obtain outside medical records to assist Health Information Department to reconcile patient medical records within scope.
  8. Access payer portal and upload data/results to close quality and health maintenance patient care gaps that are open.
  9. Assist with scheduling patient appointments as needed.
  10. Analyze documentation and coding of encounters for charge capture and diagnostic coding training opportunities.
  11. In collaboration with the coding team, provide education and training for providers and care teams.
  12. Prepare, review, and audit the data collected for accuracy and completeness.
  13. Monitor revenue integrity of payer payments in accordance with contract.
  14. Participate in interrater reliability and auditing programs that ensure uniform and accurate data capture.
  15. Proactively seek opportunities to improve abstraction and reporting efficiencies.
  16. Follow the policies, procedures, and practices outlined in the Code of Conduct and Organizational Ethics policy. Report any business practice/compliance concerns according to organization policy.
  17. Performs other duties as assigned.

License, Certification, Education or Experience:

REQUIRED for the position:

  • High School diploma or equivalent
  • Current WA State Medical Assistant Certification
  • Two years of experience in health care, acute care hospital or ambulatory clinic
  • Electronic medical record (EMR) experience
  • Knowledge of Microsoft Office products such as Word, Excel, and Power Point
  • Knowledge of medical terminology
  • Experience with CPT, ICD-10, and HCC coding for outpatient/clinic setting.
  • Experience with HEDIS and other national metrics.
  • Exceptional organizational skills
  • Demonstrated ability to meet deadlines while working on multiple projects.
  • Excellent communication skills both orally and in writing.
  • Ability to communicate and interact with individuals and groups representing all levels of the organization, and business partners.
  • Strong attention to detail, accuracy, and ability to work independently.
  • Exhibits dependability and good judgment skills.

DESIRED for the position:

  • Relevant data management and data analysis experience, including use of statistical methods
