• Deliver comprehensive clinical behavioral health services in alignment with CHDC’s mission and quality standards, focusing on boosting patient satisfaction and health outcomes.
• Assist patients in navigating the complexities of both physical and behavioral healthcare systems to ensure integrated and holistic health care.
• Educate individuals about available resources and services that foster participation in disease prevention, wellness, and overall healthy lifestyle choices.
• Participate actively in multi-disciplinary treatment team meetings to coordinate care effectively with healthcare professionals and social service providers.
• Report any psychiatric or medical crises immediately to the Chief Medical Officer (CMO) and document unusual incidents promptly to ensure patient safety.
• Maintain precise and comprehensive documentation for clinical records as well as comply with designated documentation practices in other areas.
• Engage in and contribute to the Quality Improvement program, attending all relevant training sessions and staff meetings to stay updated and enhance service delivery.
• Implement evidence-based screening tools to assess patients' healthcare needs and summarize findings appropriately.
• Collaborate closely with patients dealing with Opioid Use Disorder as part of our Center of Excellence Team, providing specialized support and resources.

This role primarily focuses on individuals from diverse backgrounds seeking mental health support, many of whom may be grappling with chronic illness and social determinants of health. By providing tailored resources and support services, the LCSW will empower these patients to engage in their care actively, enhancing their overall health literacy and access to necessary treatments.

• A master’s degree in Social Work is required, alongside a current Licensed Clinical Social Worker (LCSW) certification.
• At least 2-3 years of practical experience in behavioral health is necessary to succeed in this role.
• Experience within a medical setting, whether in primary care or specialty care, will be beneficial and is preferred.
• Must demonstrate the ability to work collaboratively with a team of healthcare professionals, social service agencies, and clients.

Further experience in integrated care settings is desirable, as is previous engagement in multi-disciplinary care teams. The willingness to travel to various CHDC sites for meetings, training, and outreach initiatives is essential to foster collaboration across the organization.
