This position is entirely onsite, engaging directly with patients in their homes. You will work within a collaborative environment that includes multidisciplinary healthcare professionals, allowing for comprehensive care. Our office hours are from Monday to Friday, 9 AM to 5 PM, fostering a healthy work-life balance for our staff.

• Conduct thorough assessments of patients’ physical abilities, limitations, and individual needs to develop customized treatment plans.
• Implement therapeutic exercises and manual therapy techniques aimed at restoring function and preventing disability.
• Work closely with physicians, nurses, and other healthcare providers to ensure seamless patient care and optimal outcomes.
• Educate patients and their families about therapy methods, rehabilitation techniques, and self-management strategies to foster active participation in their recovery.
• Maintain accurate and up-to-date patient records, documenting interventions and tracking progress through standardized measurement tools.
• Stay current with the latest evidence-based practices and advancements in physical therapy, contributing to professional discussions and training sessions.

Your primary focus will be on patients receiving home care services, which can include a diverse range of individuals dealing with post-surgical recovery, chronic pain, neurological disorders, and rehabilitation following injuries or strokes. You will play a critical role in their recovery journey, tailoring interventions that respect their living environment and personal circumstances.

• A Bachelor’s Degree in Physical Therapy from an accredited institution.
• Current licensure as a Physical Therapist in the state of California.
• In-depth understanding of human physiology, anatomy, and the principles governing effective physical therapy practices.

• A minimum of 2 years of relevant experience as a Physical Therapist, preferably in home care or outpatient settings.
• Experience in pediatrics or acute care environments is a plus, although not mandatory.
• Strong communication and interpersonal skills to effectively engage with patients and their families.
• A collaborative mindset, with the ability to work both independently and as an integral part of a healthcare team.
