Full-Time Registered Nurse (RN)

Springpoint At Home is seeking an experienced Full-Time Registered Nurse (RN) to join our team!

Springpoint at Home provides private duty home care services. We are seeking a Full-Time RN to support our Mercer and Middlesex communities. Our RNs are responsible for admissions, on-going client assessments and supervising CHHA's in non-skilled level of care. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations.

Responsibilities:

  • Oversees medication administration for clients in independent living for the on-site clinic.
  • Responsible for the supervision, instruction, support, and coaching of CHHA staff on providing quality assured home care services in an organized and cost-efficient manner.
  • Actively seeks referrals within the community and visits/calls prospective clients and families to encourage/facilitate intake process.
  • In the absence of an Intake Schedule Coordinator, performs and/or delegates associated tasks.
  • Adhere to corporate policies regarding pay adjustments and seek Director approval for any incentives/travel time/investigation time off pay or any 'non-productive' payments.
  • Performing initial assessments within regulation timelines, including care planning and home safety assessments.
  • Making 30-day phone calls to monitor client's health, discuss performance of aides, and update care plans (or more frequently as needed).
  • Make 60-day client visits to perform re-assessments and observe/supervise home health aides.
  • Ongoing monitoring and advocating clients' need and caregiver compatibility.
  • Reviews client records for appropriate documentation at admission, recertification, and discharge.
  • Conducts regularly scheduled multi-disciplinary client/case review conferences and is ongoing as needed.
  • Monitors and ensures coordination and implementation of established patient care plans, discharge planning, and related activities.
  • Is available and a resource to staff in matter of policies, procedures, regulations, and coverage issues and/or directs staff to appropriate resources to solve the problem.
  • Directs clinical staff toward appropriate and available Springpoint and community resources.
  • Supervises and ensures the timeliness, quality, and compliance standards of documentation of assigned staff.
  • Collaborates with the scheduling team regarding staff utilization, retention, engagement, and schedule fulfillment.
  • Monitors staffing patterns and needs using performance indicators and maintains appropriate staffing levels to assure timeliness and quality of services provided.
  • Maintains an open line of communication with all levels of staff.
  • Assists staff in orientation and staff development/in-service programs and monitor continuing educator efforts by CHHA staff as per regulations and policies.
  • Coordinates efforts with other members of the management team to ensure ongoing development and improvement of agency-wide activities, including participation in Springpoint at Home's Quarterly Quality Improvement Planning meetings.
  • Oversees infection control policies and procedures and any audits of such for Springpoint at Home.

Education and Experience:

  • High school Diploma or equivalent
  • Valid NJ RN License
  • CPR Certified
  • 2+ Years experience in LTC or sub-acute setting preferred
