Nurse Supervisor (RN) Full-Time position

Location: Dallas Fort Worth, Texas (within licensed service area)

Position Overview Collage Home & Community is seeking an experienced Registered Nurse (RN) to serve as a Nurse Supervisor supporting our Texas Licensed Home Health Services (LHHS) operations. This role will initially begin as PRN and is expected to ramp quickly to a full-time position as new clients are onboarded and service volume grows. This is an excellent opportunity for a clinically strong RN who wants to help build and scale a high-quality, compliance-driven home health program focused on medically complex patients.

Role Structure (Important) Initial Status: PRN (flexible scheduling) Expected Transition: Full-time as patient census increases Ramp Trigger: Growth in skilled nursing and CNA-supervised cases Supervision Model: RN-led clinical oversight under Texas LHHS regulations This position is intentionally structured to allow the right candidate to grow into a long- term leadership role as the program expands.

Key Responsibilities Clinical Supervision Serve as the supervising RN for nursing and aide services provided under the agency’s Texas LHHS license Provide clinical supervision of RNs, LVNs, and Certified Nurse Assistants (CNAs) Ensure CNA services are appropriately assigned, delegated, and supervised by nursing Assessment & Care Planning Conduct or oversee initial and ongoing patient assessments

Develop, review, and update physician-ordered plans of care Monitor patient progress and respond to changes in condition Compliance & Quality Ensure services are delivered in compliance with Texas Health & Safety Code Chapter 142 and 26 TAC Chapter 558 Maintain accurate, timely, and complete clinical documentation Participate in audits, surveys, and quality improvement activities Collaboration & Growth Support Partner with intake, operations, and leadership to support safe onboarding of new clients Help establish scalable clinical workflows as census grows Serve as a clinical resource to staff and leadership

Qualifications Current, unrestricted Registered Nurse (RN) license in the State of Texas Minimum of 2 years of nursing experience Home health, community-based, rehab, or complex care experience preferred Strong knowledge of clinical supervision and care planning Familiarity with Texas home health regulations preferred Current CPR certification Valid Texas driver’s license and reliable transportation
