Job Summary: Registered nurses (or RNs) work closely with supervising physicians to assess and monitor patientsโ€™ needs. They ensure that treatment and care are conducted according to the procedures of the facility, as well as administer medications and perform other related tasks.

  • Intake, Care Plan, & Service Agreementย 
  • AxisCare Data Input (Care Plans, Service Agreements & Assessments)
  • Clinical Quality Reviewsย 
  • Supervisory Visits Review
  • Training for Newly hired Staffย 
  • Discharge Planning
  • QI Documentation Reviewย 
  • Healthcare Patients/Plansย 
  • Change of Conditions Recommendationsย 
  • Provide palliative care as needed to keep the clientย comfortable
  • Make recommendations for devices or tools thatย might improve the clientโ€™s quality of life
  • Listen to clients and be prepared to respond toย concerns or requests
  • Continuously educate clients, their families andย other caretakers on proper home health careย strategies and procedures

Requirements:

  • Current nursing license in the state
  • Current CPR (American Heart Association)ย Certification
  • Access to reliable transportation
  • Ability to work independently and with directions

  • Experience with Microsoft Office, Excel, Outlook andย Word to document and track client care activity

  • Initial care plans are completed in a timely matter as listed in policies and procedures Abuse, neglect on explanation are reported to Administratorsย 
  • Coordinator services with administrator and Office Managerย 
  • Proud Quality Customer Services during the visitย 
  • Make recommendations accuracy to change of condition
  • Quarterly quality improvement documentation are complete and reviewed we documentedย 
  • 100% of staff administering medication are Proxy Trained in every departmentย
