Job description

Intake / Admissions Nurse

Responsibilities and Duties

  • Facilitate admissions for incoming Skilled Nursing patients (does not include direct patient care)
  • Verify and enter physician orders
  • Identify level of care needed
  • Receive and give verbal orders accurately
  • Review clinical documentation to determine Skilled Nursing placement
  • Exceptional interpersonal and customer service skills
  • Ability to communicate clearly with medical providers
  • Work closely with referring hospitals, physicians, skilled nursing facilities, patients, and families
  • Manage after-hours and weekend referrals (rotating on call)

Schedule

  • Monday – Friday (day shift)
  • Rotating weekends and on-call (approximately every 6 weeks)

Pay: $28.00 – $32.00 per hour DOE
