JOIN OUR TEAM AS AN PRN INTAKE & ASSESSMENT CLINICIAN!

(PRN) 36 hrs/week!  + GENEROUS SHIFT DIFFERENTIALS (weekday, overnights, weekend days & weekend overnights)

Your Work Matters How will you make a difference? 

We seek a dedicated, detail-oriented Intake & Assessment Clinician to join our LBH team!

The Intake & Assessment Clinician manages referrals and service requests, assesses individuals of all ages, and collaborates with physicians for appropriate care decisions. They assist with discharge planning under the Director of Admissions and Referrals and are the primary community contact, providing essential resources and support. Additionally, they support the case management program.

  • Document and process inquiry calls according to Intake policies.
  • Conduct patient triage for appropriate placement internally or referral to community resources.
  • Assess and stabilize patients before discharge or coordinate hospital admission arrangements.
  • Notify and assist the on-call doctor with patient interviews.
  • Obtain administrative approval for all admissions.
  • Complete admission paperwork following hospital policy.
  • Maintain an intake log with all necessary information documented.
  • Make follow-up calls to patients who fail to show up for appointments.
  • Demonstrate guest relations by example and counsel employees in need of training.

MUST Work 36 hours per week (12-hour shifts)

Your Experience Matters What we’re looking for:

  • Education & Licensure: A Master’s Degree in a human relations field is required. Licensed to practice in Illinois as a mental health professional (Social Worker, Counselor, or RN with MHP). 
  • Experience: (2) or more years of experience in intake assessment activities within an acute psychiatric setting. 
  • Additional Skill Requirements:  A working knowledge of the latest DSM and Mental Status Exam and working knowledge of the Addiction and Recovery field.
