Director of Social Work

Join our dynamic team at Woburn Rehabilitation and Nursing Center, a 140-bed facility committed to providing exceptional care and services to our residents. As a Director of Social Work, you will play a vital role in planning, developing, and implementing social service programs that cater to the unique needs of our residents.

Responsibilities:

  • Plan, develop, organize, implement, evaluate, and direct the social service programs

  • Build relationships between residents and staff to support residents' individual needs

  • Find options that most meet the physical and emotional needs of each resident

  • Maintain contact with family to report changes in health, current goals, and care planning

  • Make referrals and obtain services from outside entities

  • Provide or arrange needed counseling services

  • Assist residents with financial and legal matters

  • Review facility policies and procedures as part of the facility's interdisciplinary team to assure compliance with state and federal regulations

Requirements:

  • LICSW required

  • Clinical supervision and management experience preferred

  • 2+ years' experience working in a SNF with short-term rehab/discharge planning experience

  • Excellent communication, documentation, and assessment skills

  • Possess a genuine concern for and interest in people
