KEY JOB RESPONSIBILITIES
Responsibilities include but are not limited to:
  • Directs Social Work Case Management in the Nursing Home
  • Plans, develops, and supervises the Nursing Home Social Services Program for Nursing Home residents
  • Conducts initial orientation of resident and residents' family regarding Ohio Veterans Homes and its policies, procedures, and programs
  • Assigns and monitors cases, evaluates work performed, adjusts grievances, orients, trains, and counsels employees
  • Attends training sessions and workshops to provide input into the Plan-of-Care and discharge planning
  • Supervises Nursing Home social workers who prepare and update social histories on all nursing home residents and all paperwork required relative to services provided
  • Directs meetings of Resident's Counsil, establishes Resident's Grievance Committee as mandated by Ohio Revised Code providing minutes of all meeting to the Nursing Home Administrator

Qualifications
Requires current license as independent social worker (i.e., LISW) as issued by State of Ohio Counselor, Social Worker & Marriage & Family Therapist Board in accordance with ORC 4757.27; 12 mos. exp. providing clinical supervision.
Pursuant to ORC 4757.41 (A)(5) any person engaging in social work or professional counseling as a civil service employee as defined in section 124.01 of the Revised Code who has at least two years of service as of July 10, 2014 is exempt from current licensure as independent social worker (i.e. LISW) requirement.
