Position Summary
Case Management:
  • Provide direct case management services to children and youth, meeting annual service delivery unit expectations, including treatment service planning, assistance in daily living, supportive services, crisis intervention, linkage/referral advocacy and support of client's self-determination.
  • Maintain/update necessary client documents, records and reports.
  • Work collaboratively with other agencies involved in client's care.
  • Work collaboratively with multidisciplinary clinical team regarding clients.
  • Complete necessary documentation and paperwork according to agency policy.
  • Provide transportation of clients in personal vehicle, as necessary.
  • Work collaboratively with school personnel.
  • Go to student's home, or the community, or other setting, if necessary and indicated.
  • Facilitate referrals and linkages to needed services.
  • Provide behaioral health consultatino and education to caregivers/parents
School Based Preventions Specialist:
  • Delivery of educational services, i.e. classroom educational groups, skill building services *Complete psychosocial assessment of student's needs and determine need for further community services.
  • Assist in establishing environmental change via advocacy.
  • Build awareness of mental, emotional and behavioral health that impact families and communities using prevention curricula.
  • Provide prevention services through interagency collaboration and networking.
  • Provide opportunities for positive behavioral support via peer leadership activities.
  • Provide brief intervention and referral for treatment, as indicated.
  • Participate/collaborate with the other Ready to Learn school staff in program/activities planning.
  • Other duties related to this position as needed and appropriate, and assigned by Supervisor.
