This position is fully remote, allowing for flexibility while working in a supportive and collaborative environment. • Conduct comprehensive assessments of clients presenting with major mental illnesses and severe emotional disorders either in their homes or community settings.
• Create, implement, and monitor integrated care plans tailored to each client's unique needs, involving families and other healthcare professionals in care coordination.
• Provide evidence-based therapy methods, including individual, group, and family counseling, ensuring that services align with the specific presenting issues and client requests.
• Facilitate crisis intervention when necessary, employing therapeutic techniques and resources to support clients during critical moments.
• Work collaboratively with a multi-disciplinary team to improve patient care delivery, actively participating in case reviews and team meetings.
• Maintain adherence to COHC’s ethical standards and professional guidelines, fostering a culture of trust, respect, and safety for clients.
• Ensure timely documentation of client interventions and outcomes, submitting required data reports in accordance with organizational timelines.
• Act as an on-call resource for clients, providing assistance and guidance after hours as needed.

The LCSW will work with clients at various stages of emotional and mental health challenges, particularly those enrolled in hospice care. This includes individuals dealing with terminal illnesses, their families, and caregivers, all of whom require holistic support and empathetic communication. Our focus is on enabling clients to navigate their emotional responses while promoting dignity and quality of life.

• Master's degree in Social Work from an accredited institution.
• Current state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of clinical experience, preferably in a hospice, palliative, or community health setting.

• Experience providing therapy to clients with complex emotional needs; exposure to hospice and end-of-life care is highly valued.
• Familiarity with integrated care models and the ability to work effectively within collaborative healthcare teams.
• Proficiency in electronic health record (EHR) systems and other tools necessary for efficient case management.
