Job Description

Are you an experienced Registered Nurse Case Manager looking for a new opportunity with a prestigious healthcare company? Do you want the chance to advance your career by joining a rapidly growing company? If you answered "yes" to any of these questions - this is the position for you!


Daily Responsibilities:ย 

Provide case management to patients within an acute setting

Discharge planning

Utilization review/utilization Management

Responsible for quality and continuous improvement within the job scope.

Responsible for all actions/responsibilities as described in company controlled documentation for this position.

Contributes to and supports the corporation's quality initiatives by planning, communicating and encouraging team and individual contributions toward the corporation's quality improvement efforts.

Responsible for comprehensive patient assessments through health care records, physician and patient interviews.

Develops, implements, and evaluates comprehensive patient care plans.

Responsible for collaboration with healthcare team to provide feedback, updates and recommendations regarding health status and plan of care for patient.

RN license in the state of NV

Hours for this Position:

Monday - Friday 8-5ย
