  • Maintain meticulous, confidential records of patient interactions and treatment progress in compliance with HIPAA regulations.
  • Collaborate closely with an interdisciplinary team of healthcare professionals, including physicians, nurses, and chaplains, to ensure holistic, integrated patient care.
  • Engage in continuous professional development by staying current with the latest research and best practices in social work and hospice care.
Patient Population / Client Focus

Qualifications
