• Conduct thorough psychosocial assessments of patients and their families to identify needs and develop personalized treatment plans that accommodate unique circumstances.
• Implement evidence-based therapeutic interventions such as cognitive-behavioral therapy, grief counseling, and crisis intervention to support patients coping with terminal illnesses.
• Maintain meticulous, confidential records of patient interactions and treatment progress in compliance with HIPAA regulations.
• Collaborate closely with an interdisciplinary team of healthcare professionals, including physicians, nurses, and chaplains, to ensure holistic, integrated patient care.
• Engage in continuous professional development by staying current with the latest research and best practices in social work and hospice care.

Our hospice center primarily serves individuals facing life-limiting illnesses such as cancer, heart disease, and neurodegenerative disorders. In addition to physical health concerns, you will address mental health needs, helping patients and their families navigate the emotional complexities of end-of-life care. Your skill in providing compassionate support will empower clients to make informed decisions, ensuring their preferences and values are respected.

• A Master’s degree in Social Work from an accredited institution.
• An active LCSW license in New Jersey, enabling independent practice.
• Strong communication skills, both verbal and written, particularly in conveying sensitive information with empathy.
• An ability to connect with diverse populations and work effectively within an interdisciplinary team.

We are looking for candidates who have a minimum of two years of direct experience in clinical social work, especially in settings related to hospice or palliative care. Familiarity with trauma-informed care practices and experience providing counseling for anxiety, depression, and grief will be advantageous. Your proactive approach to problem-solving and collaborative spirit will enhance our overall team dynamic.
