Home Health Marketing Liaison / Community Liaison

Infinity Care Services, Inc., Home Health & Hospice is looking for an experienced Home Health Marketing Liaison / Community Liaison to join our growing team.

This is a full-time field marketing position focused on building strong relationships with healthcare providers, discharge planners, case managers, skilled nursing facilities, assisted living communities, clinics, and community partners throughout San Joaquin County and surrounding areas.

Pay:

$75,000โ€“$95,000 per year salary, depending on experience

Plus performance-based bonus opportunities

Position Overview

The Home Health Marketing Liaison will help grow awareness of Infinity Care Services, Inc. Home Health & Hospice by developing professional referral relationships, educating community partners about our services, and supporting smooth communication between referral sources, intake, and clinical teams.

We are looking for someone professional, motivated, organized, and experienced in home health, hospice, healthcare marketing, or medical sales.

Responsibilities
  • Build and maintain relationships with physicians, hospitals, discharge planners, case managers, SNFs, assisted living communities, clinics, and other healthcare partners.
  • Educate referral sources and community partners about our home health services, admission process, and patient care standards.

  • Develop and manage a territory plan for assigned areas.
  • Conduct field visits, networking, community outreach, and approved marketing activities.
  • Work closely with intake, clinical staff, and leadership to support timely communication and smooth patient transitions.
  • Document marketing activities, referral source contacts, followups, and territory updates.
  • Represent Infinity Care Services professionally in the community.
  • Follow all company policies, HIPAA requirements, Medicare guidelines, and healthcare compliance standards.
  • Qualifications
  • Home health, hospice, healthcare marketing, medical sales, or community liaison experience preferred.
  • Existing healthcare referral relationships in San Joaquin County or surrounding areas preferred.
  • Strong communication and relationshipbuilding skills.
  • Self-motivated and comfortable working in the field.

  • Organized and able to document activity consistently.
  • Valid driverโ€™s license, reliable transportation, and current auto insurance required.

  • Must be able to pass required preemployment screening, background check, and onboarding requirements.
