Introduction to the Company

Home & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients.

The Position:

Providing shift care for a 16-year-old patient living in Shasta County.


Who You Are:

You are an LVN who takes joy in improving quality-of-care to patients.

You enjoy providing one-on-one care for your patients to keep them safe, healthy and independent.

You are an honest self-starter who can take initiative and work with minimal supervision.

You communicate effectively with other team members and report any patient care issues to your supervisor timely.

You want to work with a team that inspires professional growth and share our priority of providing quality case management and home health care to the community.

Responsibilities:

  • Make visits to patient who is receiving care at clients home and or school.
  • Accompany the patient to and from school via bus, staying with them on the bus and or at school
  • Perform nursing assessments
  • Patient and family education as well as participation with school personnel in emergency evacuation procedures in the event of an emergency i.e. fire, flood, intruder.
  • Bowel care
  • Medication management and administration
  • Maintain timely and compliant clinical records
  • Document your care on an IPAD provided by the Home Health Care Management (HHCM)

Need familiarity or willingness to train on how to use the following DME/equipment:

  • G tube pump (infinity feeding pump) - replaced by HHCM RN every 3 months
  • Ventilators understand purpose of ventilator and will be trained by HHCM RN on settings, of ventilator and trach care.
  • Nebulizer
  • Hoyer Lift to move patient from bed to chair.
  • Suctioning of patient with a trach
  • Oxygen settings with a patient on a trach, and use of a pediatric ambu bag
  • Pulse ox
  • Wheelchair
  • Clearance airways shake vest for pulmonary hygiene


Essential Requirements:

  • Current California State license as a Licensed Vocational Nurse (LVN)
  • One year experience as a home health care provider preferred, would accept a new grad with a willingness to participate in a robust new grad training program for one year.
  • Valid California Drivers License, and eligible to be insured under our liability policy with a clean DMV report
  • Your own vehicle and proof of current auto insurance
  • Current CPR certification
  • Skilled in operating a computer and or IPAD, utilizing a variety of standard software, MS Office with Excel and Word, Outlook, and electronic medical records


Preferred Skills, Knowledge and Certifications

  • Ability to work with others, at all levels within the organization, and collaborate effectively.
  • Above-average interpersonal, problem-solving, and written and oral communication skills.


Service Region/ Location:

This is not a remote role and requires you to report directly to your patients home for your shift.


Schedule:

4 days a week: Monday, Tuesday, Thursday, Friday (flexible schedule preferred in case there is a need for a Wednesday or weekend day)


10 hours days from 7:30am to 5:30am may accept an 8-hour shift if 10-hour shifts are not acceptable to the LVN provider.


Travel:

You will need your own car to travel to and from the clients home in Shasta county.

The patient is incontinent, uses diapers, and does not use a catheter. The client is not ambulatory, uses a lift and a wheelchair. The client uses hearing aids. The client is nonverbal, infantile behavior and communication.

The client has a history of seizures and there are seizure protocols in place.

The mother is supportive. She would like to have someone outgoing, able to do medication administration, and personal hygiene care. She would like someone who will provide daily check-ins/progress/summaries.

Physical Requirements:

Job offers are conditional upon successfully passing a pre-employment medical exam that tests amongst other things the ability to move, visual acuity, and hearing acuity. Physical requirements for this role specifically include:

  • Sits, stands, bends, lifts, walks, and moves intermittently during working hours
  • Able to lift 35 to 50 pounds without assistance
  • Able to drive to and from the patients home


Compensation & Benefits

Pay $30.00 per hour.

  • You will be reimbursed for the mileage you travel in your own vehicle to participants home.
  • You will receive a monthly phone stipend.
  • On the first of the month following 2 months of employment, you will be eligible for health insurance, dental insurance, vision insurance, life insurance, long term disability insurance as well as Flexible Spending Account (FSA).
  • We offer 401(k) matching up to 4% after 1 year of employment and 1,000 hours worked.


Leave accrues starting on your first day of employment, and you can take paid vacation, bereavement, sick and holiday leave after 90 days.
