Job description

Hospice Registered Nurse

The Registered Nurse (RN) will provide direct patient care services to hospice patients and supportive care to the patient's family. The RN will manage an interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff to enhance the quality of hospice care. The RN will case manage all assigned patient clinical records. The RN assures the highest quality of care is provided and documented in accordance with the Plan of Care. The RN performs initial and ongoing assessment of the patient. The RN helps develop and implement the Hospice Plan of Care as prescribed by the physician and in conjunction with other members of the interdisciplinary group. The RN works closely with other members of the interdisciplinary team to provide comprehensive physical, psychosocial, educational, emotional, and spiritual services, including bereavement support to patients and families. The RN reports any change in the patient's condition to both the attending physician and hospice physician. The RN maintains accurate and updated medical records, including all interdisciplinary notes and Plans of Care. The RN practices infection control measures and standard precautions. The RN monitors the scheduling of all staff. The RN participates in the Quality Assessment and Performance Improvement Program. The RN may be requested to participate in an on-call schedule. The RN consistently promotes core values. This is a safety-sensitive position. The RN completes required compliance annual training. The RN performs other activities as assigned and attends staff meetings. The RN maintains a high level of knowledge pertaining to the management of pain and symptoms in the dying process. The RN makes routine supervisory visits with all other team members at least monthly. The RN performs ongoing reviews and follow-ups on assigned patient clinical records to assure quality services are provided and standards of practice are followed. The RN maintains close contact with the patient's family and/or significant others to provide information, support, and continuity of care. The RN maintains primary control and professional management of each patient and acts as the primary liaison between physician, patient/family, and the hospice team. The RN is responsible for the education of the patient, family, or other caregivers regarding needs identified on the Plan of Care, including medication administration and care of the terminally ill patient. Required skills include patient care, hospice care, case management, patient assessment, and plan of care development. Additional skills include written communication, transportation for work, interdisciplinary collaboration, bereavement support, medical record maintenance, infection control, quality assessment, pain management, symptom management, patient education, medication administration, dressing changes, and oxygen administration. Knowledge of community resources, oral communication skills, documentation, program management, and regulatory compliance is also essential.
