RESPONSIBILITIES:
  • Perform home visits for patients in a home care setting
  • Observes, records, and coordinates care with multi-disciplines regarding the patient's treatment and any change in condition
  • Teaches, supervises, and counsels the patient and family regarding home therapy procedures and other care needs as appropriate to the patients condition

QUALIFICATIONS:
  • Current license as an RN in Connecticut
  • Valid drivers license
  • Reliable transportation and willing to travel in New London County
  • Strong communication and interpersonal skills
  • Home health experience including Medicare preferred
