Internship/Residency/Fellowship: Completion of an accredited residency in Otolaryngology Head and Neck Surgery approved by the Accreditation Council for

Graduate Medical Education (ACGME)

Certifications: BLS& ACLS.

Board Certification: Board Certified or Board Eligible by The American Board of Otolaryngology Head and Neck Surgery

Licensure: Current, full, active, and unrestricted license to practice medicine in Nevada.
