Active In-Home Therapy is a leading provider of home health services committed to delivering compassionate and high-quality care to individuals in the comfort of their homes. We are currently seeking a skilled and dedicated Home Health Physical Therapist Assistant to join our dynamic team.

Position Overview:

As a Physical Therapist Assistant, you will play a crucial role in supporting physical therapists in providing specialized rehabilitation services to our patients within the familiar and supportive environment of their homes. You will assist in implementing personalized treatment plans and therapeutic exercises to promote recovery, improve mobility, and enhance overall well-being.

This is a PRN position NOT full-time, meaning you will be working with us on an as-needed basis.

You will be hired as a W2 employee, not as an independent contractor.

We offer flexible scheduling, and you can freely choose the areas you prefer to cover.

Key Responsibilities:

  1. Assist in conducting assessments of patients' physical conditions under the supervision of a licensed physical therapist.

  2. Implement individualized treatment plans developed by the physical therapist.

  3. Provide hands-on therapeutic interventions to improve patients' strength, flexibility, balance, and coordination.

  4. Educate patients and their families on therapeutic exercises, mobility aids, and strategies to promote independent living.

  5. Monitor and document patients' progress, reporting any changes to the supervising physical therapist.

  6. Collaborate with other healthcare professionals, including physicians, nurses, and occupational therapists, to ensure comprehensive patient care.

  7. Maintain accurate and up-to-date patient records.

  8. Communicate effectively with patients, families, and caregivers to ensure a positive and supportive therapeutic environment.

Qualifications:

  1. Associate's degree from an accredited Physical Therapist Assistant program.

  2. Current state licensure as a Physical Therapist Assistant.
