Position Summary
The Wellness RN plays a key role in promoting client health, safety, and well-being through in-home wellness visits. This role is ideal for a nurse who values autonomy, meaningful one-on-one care, and a predictable 4-day-per-week schedule.
Responsibilities
  • Conduct in-home assessments and routine health evaluations
  • Monitor vital signs, medication compliance, and overall client status
  • Provide education to clients and families on health maintenance and safety
  • Collaborate with the care team to support individualized care plans
  • Identify changes in condition and communicate promptly with supervisors
  • Promote independence, dignity, and quality of life for each client
