OB Nurse Navigator

A Brief Overview
The OB Nurse Navigator functions as part of a multidisciplinary team and utilizes independent decision-making skills to effectively collaborate with patients and providers. Guided by the Triple Aim Framework, this role focuses on reducing system fragmentation and improving obstetrical care coordination. The OB Nurse Navigator integrates a biopsychosocial approach-addressing medical, behavioral health, and social needs-into comprehensive care planning. Additionally, the role partners collaboratively with care coordination teams to plan and implement wraparound services that support patient care.
What you will do

  • Oversee and coordinate care across the full continuum of obstetrical and perinatal services.
  • Manage and facilitate perinatal appointments using both traditional and non-traditional methods (e.g., in-person and telehealth).
  • Serve as a liaison between patients and providers to ensure effective communication and care coordination.
  • Develop and deliver perinatal health education, including classes for diverse audiences.
  • Support community outreach through participation in screenings and educational events.
  • Coordinate medication access in collaboration with patients, providers, and case management.
  • Promote clinical competency by facilitating validation processes and supporting staff proficiency.
  • Maintain accurate, complete, and timely documentation within designated electronic record systems.
  • Empower patients in perinatal self-management through relationship-building, follow-up, and resource provision.
  • Monitor, track, and report performance metrics and quality outcomes while ensuring use of evidence-based, health-literate materials.
  • Participate in SBU, dyad partnership, staff and department planning meetings
  • Participate in community or hospital committees as assigned


Education Qualifications

  • BSN Required


Experience Qualifications

  • Five (5) or more years' experience as a clinical RN or related experience Required
  • Prior L&D experience Preferred
  • Prior experience in community health education or public health Preferred
  • Prior experience as a Health Educator Preferred


Skills and Abilities

  • Ability to work independently, adjust priorities often and establish rapport quickly
  • Demonstrated experience in diplomacy and ability to contribute to team goal setting and program development
  • Proficient PC skills and experience of Windows and Microsoft office programs required.
  • Exemplary level of core customer service skills and competency


Licenses and Certifications

  • Registered Nurse - South Carolina Department of Labor, Licensing and Regulation Required
  • Basic Life Support Certified - American Heart Association within 30 Days Required
