POSITION SUMMARY
The Registered Nurse Case Manger (RNCM) coordinates and directs the patient's care based on individual patient needs. This position is responsible for independent management of the home health patient population requiring the use of advanced assessment, teaching, and decision-making skills. The RNCM is responsible for ensuring that appropriate referrals to other services are made, interdisciplinary conferences take place regularly, and appropriate documentation is completed. Work schedules are determined to meet patient care needs and work may be required on weekends and holidays. Relevant knowledge and experience are consistently applied to new patient populations.
ESSENTIAL JOB KNOWLEDGE

  • Performs comprehensive assessments for physical, functional, psychosocial, and cognitive status of the home health patient utilizing interview, observations and physical exam techniques.
  • Plan with the patient and family for care which is feasible within the physical, financial, and functional resources of the family.
  • Establishes individualized, measurable goals in consultation with the patient, family, and other health care providers.
  • Works collaboratively with the inter-disciplinary team and multiple community resources to best meet the needs of the home health patient and family.
  • Evaluates patient's responses to care based on regular assessment and analysis of nursing intervention.
  • Maintains an updated HER record on each patient at all times, meeting required regulatory and clinical requirements.
  • Acts as a role model for professional nursing and allied health students, accompanying them on home visits as assigned.
  • Recognizes the importance of respecting individual patient and family choice regarding care and treatment options.
  • Successfully completes initial and ongoing organizational and department education, training, and competency requirements.
  • Infection Control: Understands, demonstrates, and follows organizational infection control guidelines (CDIC).
  • Quality: Focuses on improving processes and champions efficiency in daily work activities.
  • Safety: Promotes a culture of safety by taking accountability for individual actions, including but not limited to, promptly reporting errors or near misses via the organization's occurrence reporting system and participating in developing improved safety processes, as appropriate. (ADM015, ADM016, OPS044, Mission Statement).
  • Performs other duties as assigned.
JOB REQUIREMENTS
Licenses:
  • State of Wisconsin Driver License is required.
  • State of Wisconsin Professional licensure for Nursing required.
Education:
  • Associate degree in registered nursing is required.
  • Bachelor's degree in registered nursing is preferred.
Experience:
  • A minimum of one (1) year of recent experience in nursing is required.
  • Experience and a strong knowledge base in home health is preferred.
Miscellaneous:
  • On call is required.
  • CPR/BLS Certification is required.
  • Valid Auto Insurance is required.
  • Reliable vehicle transportation is required.
