Pay range for this position is $36.59 - $53.17/hour. Actual pay is based on years of licensure. This is an Hourly position.
Overview
POSITION SUMMARY: The RN Home Health collaborates with the RN Case Manager to develop and implement the home health plan of care. The RN Home Health communicates with the patient's provider and other members of the care team such as nursing, therapy, social work, home care aide, respiratory therapy, home medical equipment and home infusion services to facilitate and expedite the best possible patient outcomes. The RN Home Health is responsible for a scope of care that encompasses all ages from infancy through geriatric and demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served. The RN Home Health provides evidence-based, patient-centered care at the patient's place of residence, using critical thinking and decision-making skills. The RN Home Health has a thorough understanding of OASIS documentation and plan of care development, and their impact on 5 Star Agency Performance and Value Based Purchasing performance.
Qualifications
REQUIRED QUALIFICATIONS:
Education: Completion of nursing program
Licensure/ Certification: IL or MI RN Licensure depending on work location.
Current American Heart Association HealthCare Provider BLS is required prior to start date of employment.
Other Skills/ Knowledge: Excellent interpersonal and communication skills.
Solid computer skills, including proficiency with Microsoft software.
Strong analytical and problem-solving skills, with the ability to be detail oriented.
Provides own transportation and auto insurance and abides by all laws, rules and recommendations for safe driving while on duty.
Maintains a valid driver's license.

Education: BSN
Experience: Experience as an RN in a healthcare environment, Home Health OASIS experience preferred. Licensure/ Certification: OASIS Certification
