DESCRIPTION

Axiom Healthcare is seeking a compassionate and skilled Registered Nurse (RN) to deliver high-quality care to residents in our skilled nursing facility. The Registered Nurse (RN) is responsible for total nursing care during the assigned shift, including oversight of staff, delegating responsibilities, and ensuring adherence to all facility policies and procedures.

Duties and Responsibilities

Provide total care for residents on assigned unit during scheduled shift as a Registered Nurse (RN)

Collaborate with allied health team members to develop and implement individualized care plans

Administer prescribed medications and treatments in accordance with policy and evaluate effectiveness

Maintain an environment that supports residents’ physical, emotional, and social well-being

Educate residents and families about care plans, treatments, and procedures

Monitor team performance and support staff development, evaluations, and corrective actions as needed

Assign and oversee nursing duties to ensure tasks are completed appropriately and efficiently

Stay current on facility procedures and participate in in-service and continuing education

Identify and respond to significant changes in residents’ conditions

Accurately document care provided, changes in condition, and other relevant information per facility guidelines

Act as a charge nurse and serve as a resource for staff, offering clinical support and decision-making guidance

Follow all facility and departmental safety protocols


Skills and Qualifications

Graduation from an accredited nursing education program

Active Registered Nurse (RN) license in the state of Illinois

No history of abuse convictions

Strong interpersonal and communication skills

Ability to follow facility policies and support quality care

Must be able to work effectively with residents of varying ages and medical needs

Must meet physical job requirements, including occasional lifting, bending, and standing
