About the Role:

We are seeking a highly skilled and compassionate MDS Coordinator to join our care team. As a MDS Coordinator, you will be responsible for coordinating the Minimum Data Set (MDS) process, ensuring accurate and timely completion of assessments, and developing and implementing individualized care plans. You will also be responsible for supporting our multidisciplinary team in providing comprehensive care to our residents.

Responsibilities:

• Coordinate the MDS process, ensuring timely and accurate completion of assessments and care plans
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Conduct assessments, gather data, and document resident information accurately and thoroughly
• Collaborate with healthcare professionals to identify and address resident needs
• Develop and maintain relationships with residents, families, and staff to promote open communication and collaboration
• Participate in quality improvement initiatives to enhance resident care and satisfaction

Requirements:
