This position offers a flexible remote work setting, allowing for a work-life balance that accommodates personal and family responsibilities. Full-time employees may work varying shifts, including days, evenings, and some weekends depending on departmental needs. Some on-call responsibilities may be required to ensure our patients receive ongoing support.

• Conduct thorough psychosocial assessments to identify patients' emotional, social, and financial needs, collaborating closely with medical teams for holistic care.
• Provide individual and group therapy sessions, focusing on adjustment issues, grief counseling, and coping strategies for patients facing health challenges.
• Coordinate with community organizations to ensure appropriate post-discharge services are arranged, facilitating continuity of care.
• Offer emotional support and education to patients and their families about treatment options and available resources to enhance understanding and compliance.
• Participate actively in hospital committees, contributing expertise on social work issues and advocating for patient needs.
• Provide crisis intervention services as required, delivering immediate support to patients in distress.
• Develop and implement comprehensive discharge plans, linking patients to necessary community resources.
• Maintain meticulous records and documentation in compliance with healthcare regulations and organizational policies.

Our patient population includes individuals from diverse backgrounds dealing with various psychological and social challenges related to medical conditions. You will primarily work with adults navigating complex healthcare situations, helping them cope with the impact of their health on their daily lives and overall well-being.

• A Master's degree in Social Work from an accredited institution.
• Current state licensure as a Licensed Independent Social Worker (LISW).
• A strong grounding in medical terminology and mental health disorders.
• Excellent communication, interpersonal, and teamwork skills that foster collaboration among multidisciplinary teams.

While not mandatory, candidates with at least two years of experience in a healthcare or hospital setting will be strongly regarded. Familiarity with crisis intervention techniques and community resource coordination is highly desirable.
