Key Responsibilities

• Conduct comprehensive psychosocial assessments to gather insights into patients' mental health concerns and needs during the hospice phase, identifying strengths and coping mechanisms.
• Develop and implement individualized treatment plans that address the psychological and social needs of patients and their families, including clear goals and measurable interventions.
• Facilitate individual, family, and group therapy sessions using evidence-based therapeutic modalities, contributing to emotional healing and resilience in the face of life-limiting illnesses.
• Collaborate with multidisciplinary teams, including physicians, nurses, and chaplains, to provide coordinated care and ensure holistic approaches to patient support.
• Offer crisis intervention services and conduct risk assessments to identify and mitigate potential issues that may arise for patients or their families.
• Maintain accurate, timely documentation and electronic health records, ensuring compliance with ethical and legal standards while securing patient confidentiality.
• Facilitate psychoeducation classes and support groups to empower families with coping strategies and tools for managing grief and health-related challenges.

Patient Population / Client Focus

In this role, you will provide care to a diverse population of patients facing terminal illnesses, including cancer, heart disease, and neurological disorders. Your work will support not only the patients but also their families, who may require guidance and resources as they navigate complex emotional and logistical challenges related to end-of-life care.

Qualifications

• A Master's degree in Social Work from an accredited program is required.
• An active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
• Demonstrated capacity to evaluate complex psychosocial situations and implement effective interventions.
• Proficiency in evidence-based therapeutic modalities relevant to hospice care.
• Exceptional interpersonal and communication skills to effectively work with patients and their families.

Preferred Experience

• A minimum of two years of experience in a mental health or hospice setting is preferred.
• Familiarity with multidisciplinary teams and experience in working remotely are highly valued.
• Experience in providing grief counseling or support through life transitions enhances candidacy for this position.

Compensation and Benefits
