Key Responsibilities

• Conduct thorough assessments to understand clients’ social, emotional, and environmental issues that may affect their recovery journey.
• Design and implement personalized service plans that outline treatment goals, strategies, and methods for tracking progress.
• Provide therapeutic counseling to individuals and families navigating substance use disorders, mental health challenges, and other life difficulties.
• Collaborate closely with clients to create tailored plans and seamlessly connect them to necessary community resources such as support groups and rehabilitation services.
• Offer immediate crisis intervention services, including risk assessments and safety planning, to clients in urgent need.
• Engage collaboratively with local agencies and organizations to coordinate services, advocating for clients and ensuring access to vital resources.
• Participate in community outreach initiatives to raise awareness about the services our agency offers and promote community well-being.
• Attend case conferences, team meetings, and ongoing training to stay informed on best practices and enhance team collaboration.

Patient Population / Client Focus

You will primarily work with individuals in a detox center setting who are struggling with substance use disorders. Your role involves providing empathetic support and guidance during a critical time in their recovery process, focusing not only on their immediate needs but also on long-term solutions to empower them beyond their addiction challenges.

Qualifications

• A Master’s degree in Social Work from an accredited institution is required.
• An active state license as a Licensed Clinical Social Worker (LCSW) is essential for this position.
• Demonstrated knowledge of trauma-informed care practices, social justice issues, and community resources is vital.
• Strong communication skills, cultural competence, and the ability to build rapport with diverse populations are critical.
• Proficient problem-solving abilities to navigate complex client situations and develop effective intervention strategies.

Preferred Experience

• Prior experience working in a social services agency or a related field with a focus on mental health, addiction, or crisis intervention is preferred.
• Familiarity with case management tools and best practices in telehealth and remote client engagement.

Compensation and Benefits
